ax+by+cz+d= 0 is given by  \frac{\left [ ax_{1}+by_{1} +cz_{1}+d\right ]}{\left | \sqrt{a^{2}+b^{2}+c^{2}} \right |}

 4x+2y+4z=16

4x+2y+4z=-5

d= \frac{\left | 6-(-5) \right |}{\sqrt{4^{2}+4^{2}+2^{2}}}= \frac{21}{6}= \frac{7}{2}
