Subject: Permission for leave for two days.

Sir/Ma’am,

I beg to state that I have been suffering from viral fever for the last two days. My doctor has advised bed rest for two days. Therefore, I request you to kindly grant me leave for the next two days with effect from 29th to 31st  March, both days inclusive. A medical certificate is enclosed along with this application. I shall be grateful to you for your act of kindness in this regard.
