During urine formation, the tubular cells secrete substances like H+,K+  and ammonia into the filtrate. Tubular secretion is also an important step in urine formation as it helps in the maintenance of ionic and acid base balance of body fluids.

Hydrogen is secreted in both PCT & DCT .

potassium is reabsorbed from PCT , but is secreted into filtrate in DCT.

Ammonia is secreted into filtrate in PCT.

Potassium is secreted in tubules and,in exchange sodium is reabsorbed. Excess Secretion of K+ and defective reabsorption of Na+ will cause a condition called hyperkalemia.

hence option C is correct.
