(a) Mutation is a phenomenon which results in alteration of DNA sequences and consequently results in changes in the genotype and phenotype of organisms.

Mutations occur due to loss (deletion) or grain (insertion or duplication) of a segment of DNA.

(b) 

Point Mutation Frameshift Mutation
Mutation due to change in a single base pair of DNA Deletion and insertions of base pairs of DNA, causes frameshift mutations.
Example - sickle cell anemia
