Carbon monoxide is formed by incomplete combustion of carbon.

carbon monoxide forms a complex with hemoglobin which is much stronger than oxygen-hemoglobin, also carbon monoxide can substitute oxygen and reduce oxygen carrying blood. 

Carbon monoxide binds with Hb to form a 300 times more stable compound than the oxyhemoglobin complex.

Therefore, Option(4) is correct.
