Subject: Request for Two weeks leave

Respected Sir,

I beg to inform you that I have been diagnosed with jaundice yesterday and the doctor has advised me of two weeks bed rest. Therefore, I kindly request you to grant me two weeks leave from 3rd March to 17th March,both days inclusive. A medical certificate has been attached with this application.

I shall be grateful to you for this act of kindness in my regard.
