SUBJECT: Application for sick leave.
Respected Sir,
I am a student in Class 9-B of your school and I beg to state that I have been suffering from fever for the past two days and have been diagnosed with jaundice. My doctor has advised me complete bed rest for the next twenty days at the least. Therefore, I request you to kindly grant me leave for the next twenty days, that is from 1st April to 20th April, both days inclusive. A medical certificate is enclosed with this application.
I shall be grateful if you do the needful.
